Your Case Study should follow the following structure:

  1. Introduction

Identify a patient/client in whose care you have been involved including a brief overview of the patient/client’s health problem, initial presentation and subsequent significant change or deterioration in health status.

Explain the scope of the case study and include a confidentiality statement to indicate that any patient details have been anonymised (in observation of GPDR and NMC requirements).

  1. Main Body

Develop the case study by critically discussing the patient’s alteration in health status, analysing patient data systematically and comparing interventions, taking current evidence-based practice into consideration. This should include evaluation and interpretation of observational and investigative data used to inform your plan of action and clinical decision- making, reasoning and judgement based on patient changes, the situation and the working environment.

Critically discuss your decision-making and justify your actions with supportive evidence.

Discuss any continuing care needs including liaison with the MDT if relevant. Include a short paragraph evaluating the outcomes of the decisions that were taken, reflecting on your clinical decision-making strategies, reasoning and judgement in order to enhance your practice delivery in the future.

3. Conclusion

Briefly summarise what has already gone before and reiterate key points on how the processes of clinical decision making, reasoning and judgement, in the case study, were supported by objective evidence.

4. Reference list

All references cited in the assignment must appear in the reference list and cited as per University guidelines. (NB the reference list is not included in the word count).

Assignment formatting guidelines:

Your student roll number should be the title of the Word file. Do not insert your name anywhere in your assignment or on Turnitin.

Type your essay in Microsoft Sans Serif or Verdana font, size 12 or larger and fully justify your formatted text. Number all pages.

Use 1.5 line spacing in the main body of your assignment and single line spacing in your reference list at the end of your assignment.

Provide a final word count (not including your reference list) at the end of your assignment.

(c) critically discuss concepts of integrated care pathways, evidence-based practice, clinical effectiveness and clinical governance in relation to effectively responding to the health needs of their specific client group.

Patient confidentiality:

The University guidelines for assignment submission are clear on the issue of protecting patient confidentiality and this is a reminder to ensure that under no circumstances do you allow identification of a patient in your submitted work.

Please be mindful that this includes the following:

  • Clinical documentation where the patient’s ID, name, address, etc is clearly visible or can be identified
  • Diagnostic information that has identifiable patient data visible

In the case of students who do not take the utmost care to protect patient confidentiality the submitted assignment may be classed as a referral.

Your client focussed case study should be no more than 3,500 words. The final reference list does not form part of the word count, but your ‘in-text’ citations do form part of the word count

Penalties for exceeding the word count

You must get as close to the word limit as possible. In accordance with the University’s Assessment and Feedback policy, ff your work is over the specified 3,500 words the surplus (those words in excess of the first 3,500) will not be considered when assigning your final grade.

Assignment Support arrangements

There are two timetabled three-hour Lecturer facilitated group supervision sessions within the module. During these sessions you will be expected to present and discuss your case study with your peer group and academic staff who will provide you with constructive feedback.

Group Assignment Supervision 1

Please come to this session with a plan of your assignment and be prepared to present and discuss the plan with your peers and lecturer

You would have identified the patient/client, can explain the background, the relevant health issue as you intend in the assignment introduction.

Group Assignment Supervision 2

In this session you will be expected to present and discuss your case study which should now be well developed though not yet complete. For example, you should be able to discuss some of the clinical decisions and judgements that were made and the reasoning behind them.

Both sessions will be facilitated by Lecturers and additional information is located on the module Blackboard site.

Reassessment

If you fail your assessment, and are eligible for reassessment, you will need to resubmit on or before 4:00pm: 16/05/2022 Feedback: 17/06/2023

For students with accepted personal mitigating circumstances, this will be your replacement assessment attempt. Students should be aware that there is no late submission period at reassessment (this includes students who have accepted PMCs from a previous attempt).

The reassessment assessment is the same as the original summative assessment task
